A 2004 fan module is probably outside of the VIN range of the recall from years ago. The module can still fail even if it is one of the updated units. I have changed almost a hundred back when the early ones failed and during the recall. The warranty time was .8 hours but you will probably need ALOT more time if you have never done one because of where it is located on the fan shroud (the wire connectors are not easy the get to)!!
