My car had a little shake at idle when I got it, but after cleaning the throttle body, MAFs, and running a can of BG 44K, it runs silky smooth.

Second this. Good idea to start with the basic maintenance items.

Also I never understood throttle body cleaning can the little bit of sludge build up effect anything? or is it the placebo effect?

These can be combined as well: Idle valve stuck open + Main flap donīt close + airflow sensor is dirty and donīt have correct measurement = Engine running lean and ECU think all is good. Add faulty or very old Lambda sensor on it and we have a perfect mess where no faulty codes but engine hardy even run.
