Transmission issues

Mostly, the transmission in my car operates flawlessly, except under a couple of specific circumstances.
First. If the car is facing downhill and is traveling at about walking pace, the transmission will change up, I'm assuming from first to second, with sledgehammer force. If the car is on the flat or facing uphill, and I'm using more throttle, the change is almost imperceptible.
Second. When merging onto a highway and need to increase speed quickly from say 40 mph to 70 mph, the transmission will sometimes screech at a gear change (4th to 5th perhaps) and the car will slow down. Once I let off on the throttle, the car will behave itself.
I changed the fluid & filter at about 137,000 miles in part due to these problems. The car now has 151,000 miles.
Does anyone know if there are any adjustments to the transmission or electronics that can be made to fix the problems, or should I starting saving for eventual transmission rebuild?
