Warm start troubles and sometimes rough idle

Hi there,
my Iron Lady (1989 V12 with the Magneti ignition, no cats) has a couple of little problems, which bother me a tad.
1. when the engine is hot/warm, she doesn't want to start, when standing for say 5 minutes. If I press the throttle a tad (say an inch), she'll come back to life and run as if nothing was wrong. If I stop and start again within say 3 minutes, no problem what so ever. Can anyone give me some advice on what to look for?
2. the V12 is always on 12 cylinders but sometimes the idle is rough. Enough to wobble the interior. All 12 cylinders are firing and when I accelerate, there is no problem then. Nor when cruising at 1500 rpm or so. As soon as the car hits idle again, no matter if in D, N or P, she'll shake again.
As said, all 12 cylinders fire. I changed the plugs and leads about 4 months ago. The trouble was before that and has been like that afterwards. I've heard that the distributor cap can cause these problems too but find it hard to believe... Can anyone advise what to do?
