Wanted to close this thread out, in the end what was causing my fuel trims to be off was a combination of
vacuum leak AND dirty intake valves, i'm not sure to what degree though.

My crossover pipe leaked during a run and while replacing it, I cleaned my intake valves with CRC cleaner and a brush.
also I replaced an O-ring located on the throttle body where the vacuum line goes in..

I suspect the vacuum line O-ring was a big factor in my fuel trims, now I am < 1% between bank 1 and 2
and have successfully remapped with great results!
