No, I was not monitoring voltage, but I can.
One of the problems is the data recorder I am using is a really low end unit only capable of sampling at 120 times per second, so it cannot resolve the 22 KHz PWM signal. Also, though the scope I have can resolve it, but does not have a deep enough memory to get a meaningful picture over a long enough time period.
I think I will try to temporarily lower the PWM frquency way down, to say 30 cycles per second for test purposes. Then I can resolve the data with the equipment that I have. Maybe then I can get some more clues and see if there is a way to predict when this is going to happen and alter the drive somehow to prevent it.

Not sure how the motor will react to pulses as slow as 30 cps. It will likely "feel" them. Maybe OK, but worth going slow here I think.
